For Medicare patients, providers should use the HCPCS code G0447 for a 15-minute visit with

the appropriate ICD-9 code ranging from V85.30 to V85.45, depending on the BMI. 

For EX : V85.30 is for BMI  of 30.0 to 30.9  (ADULT)

Medicare not covered for these ICD code range : V 85.0  to v85.25

The patient must be having the competent at the time of counseling, which may be provided by primary care physicians, advanced practice nurses and physician assistants.
